Case management and individual packages of care services

For people who require a planned and managed group of services that may include management of personal affairs, emotional support, daily living support, in home respite and transport, Catholic Community Services provides case management and packaged care. The packages of care are flexible and are adapted to changing needs with the ongoing goal to maintain or improve quality of life for each individual and their family.

Case management assists people who are unable to access and coordinate services and/or do not have a family member who is able to help them do this. Case managers have a broad knowledge of the range of services and support options available. Our case managers work with people to build on their personal strengths and identify and achieve their goals.
