Case management and individual packages of care services

Justine is a quadriplegic with partial arm function only. The case manager worked with Justine to develop personal goals with the prime goal to regain independence. Sub goals included:

  1. Help to regain wrist and shoulder function, following wear and tear damage
  2. Independent transportation to be able to drive again
  3. Vocational support to find employment

Some of the challenges for Justine were financial and emotional, as she suffers depression.

Strategies to assist Justine included:

  1. Linking her with medical support
  2. Accessing hand controls and
  3. Providing her with a computer and information about local community colleges and TAFE training

The outcome for Justine is that she now has medical options regarding the management of her shoulder injury, hand controls have been installed in her car and she has now enrolled for study in accounting. Case management support will be withdrawn over time.
