Transitional care - supporting clients to regain independence following poor health and/or hospitalisation
Intervening at the right time and providing support to people whose medical condition is starting to worsen enables them to continue to live and be cared for in their own home.
For those people that are hospitalised - intensive short term support following discharge can ensure they are able to regain independence. At this stage some people will not require any further care, though this assessment is undertaken with each person and individual needs assessed.
Working in conjunction with local hospitals, Catholic Community Services liaise closely with clinicians and social workers to ensure timely and appropriate care.
