Catholic Community Services Continuous Improvement Process aims to capture all problems, hazards, ideas and suggestions.

Clients, Carers, Family, volunteers and employees are all encouraged to provide feedback. This form is intended for use by persons who either receive or provide services by Catholic Community Services and is not for general public use.

Catholic Community Services believes that it can only provide quality services if all stakeholders are offered the opportunity to provide feedback.  

We would encourage you to use this form to provide us with details of any issue or suggestions you have about the services we provide. We in turn will provide feedback to you about your suggestion or issue, either in person or via newsletters or forums if appropriate.
