Ongoing Packaged Care and Support

A critical element to addressing homelessness is ensuring existing program and services recognise the particular needs of clients and are inclusive. These Aged care packages are specifically targeted to support the older homeless and include;

• Community Aged Care Packages that provide case management and support services for those at risk of homelessness.

• Community Options Packages for those with complex needs who experience social and financial disadvantage and those at risk of homelessness.

• Links and transition to Extended Aged Care Packages and Residential Care as individual needs change to ensure continuity of care.

We are funded to provide our services in many parts of metropolitan Sydney with a focus on reaching those within our communities who are the most socially and financially disadvantaged. We are committed to developing new and innovative projects that focus on all aspects of wellbeing to enable individuals to gain better access to housing, health, social and quality of life outcomes.
