Home and Community Care (HACC) through Ageing Disability & Home Care (ADHC
Target Group
HACC eligible people which includes people with a disability, people over 65 or someone who is a carer for someone in these groups.
What is the aim of the program?
To assist people who are living with clutter and mess that is not squalor, but does affect their daily living and functioning in their homes.
Someone who is not able to receive assistance from services due to the poor state of the home.
What service does the program provide?
It provides support and assistance for individuals who are willing to re organise and clean their homes so that they can live in a safe and clean environment.
What is the program’s assessment and review process?
Assessment is through a number of home visits, depending on client depends on amount of home visits. At time of assessments, a number of questions and paperwork filled out from Tier one documents including the ONI.
As it is short term program, review is done regularly from first service provided. Exit interview at end of service.
When does the program operate?
Services provided weekdays. Different days depending on service. Program runs throughout the whole year.
What partnerships does the program have?
Living Conditions Program works closely with Housing NSW, local mental health departments as well as ACAT and local councils.
Program Fees
No cost is required to receive service but clients are able to contribute to costs if the funding is available.
Who can refer to the program?
Self referral, family, friend, mental health teams, Councils, other agencies or anyone in the community that has concerns for someone.

Any other information
All clients that accept LCP are required to be present for all services. No decisions are made without the client’s approval.
All clients that accept LCP must also agree to ongoing support that LCP can refer on their behalf. This is necessary to either maintain or teach the client better living skills.
Referral Criteria:
Who is eligible?
HACC eligible people who are over 65, people who have a disability or the carer of someone over 65 or on a disability pension.
Who is not eligible?
People that do not fit the above criteria
Is priority access given?
All clients generally when assessed are high need therefore it relates to referral order.
