Target Group

HACC client group.

Frail aged people 65+ in need of an integrated range of basic maintenance and support services;

People with a disability who in the absence of basic maintenance and support services are at risk of premature or inappropriate admission to long term residential care;

Carers

What is the aim of the program?

The aim of the program is to assist isolated individuals to reengage with the community through information and referrals to appropriate services in the community, one-on-one counselling and emotional support, and support groups which encourage participants to form friendships and improve their confidence and skills.

What service does the program provide?

Counselling face to face issues with grief /loss or relationships, issues relating to being a carer or as the reciprocate.

Support with coping & needs based.

Information on local services for clients and/or carers. Advocacy refers to provision of advice to clients about their rights & responsibilities when receiving HACC services and at the client’s request, to act on their behalf with other service providers.

Support Groups for clients that are not ready to join main stream groups in the community due to challenging behaviour, lack of social skills.

Groups are held at Shalom and outings that provides educational information & support.

What is the program’s assessment and review process?

Initial assessments are conducted on the first visit to the client, reviews are conducted by the coordinator at intervals decided on during the initial assessment.

No cost to clients

Who can refer to the program?

Self referrals are accepted as well as referrals from external organisations and internal programs.

How do you refer to the program?

All referrals need to be processed through the intake and assessment line: 1800 225 474.

Any other information

CAS Support groups one on one or in a larger group for clients with special needs designed to prevent clients form prematurely progressing to higher level of  service.

Individuals who do not meet HACC criteria
