Community Aged Care

Ageing is part of our life cycle. Catholic Community Services sees being supported to maintain independence through our life cycle as a right. Supporting older people to continue to actively engage with their families and communities builds stronger lives and more inclusive communities.

Our philosophy of care follows a strengths based approach, supporting the social, psychological, physical and spiritual well being of clients, their carers, families and communities.

Working with State and Federal Government agencies such as the Departments of Ageing, Disability and Home Care; Health and Ageing; NSW Health; Housing NSW; and Families, Housing Community Services and Indigenous Affairs; Catholic Community Services designs, develops and delivers innovative programs and services that support clients to maintain their health and well being.

Across NSW we work with Aged Care Assessment Teams, Home and Community Care provider groups, hospitals and other community care providers to deliver and advocate for high quality client focussed care.

In supporting clients to maintain independence, we provide the following continuum of services:

  1. Information and advice about the type and range of services available
  2. Community education, awareness and advocacy - to build a community that recognises, values and supports older people
  3. Support for clients to reconnect and re-engage with the local community
  4. Support with activities of daily living
  5. Case management and packaged care - supporting clients with more complex needs ranging from low care to high care
  6. Transitional care - providing short term intensive support for clients at risk of hospital admission or transitional support for clients on discharge from hospital
  7. Support for carers
