Transpac

Program Name: Transpac (Transitional Aged Care)

Brief Description of Program:
The Transitional Aged Care Program (TRANSPAC) is a service funded to assist older people who are being discharged from hospital. The service provides older persons with the opportunity to improve their capacity for independent living and functioning and to assist them to make appropriate long-term care arrangements.

Referrals can come from any Central Coast Hospital to the Aged Care Assessment Team (ACAT) to assess eligibility for a TRANSPAC. If eligible, the Aged Care Assessment Team will then send the referral to Catholic Community Services to provide a TRANSPAC.

The range and mix of services that can be provided include:

 

  • Personal care
  • Medication management
  • Assistance with meal preparation
  • Transport to therapy or medical appointments
  • Light domestic assistance
  • Nursing support
  • Low intensity therapy to maintain mobility
  • Assistance to maintain activities of daily living
  • Case management

Eligibility Criteria: Older persons (generally over 70 years of age) who are preparing for discharge from hospital who require support to complete their restorative process.

Assessed by ACAT as being suitable for a TRANSPAC.

Cost to Clients: The fees are determined by the TRANSPAC Guidelines which require clients to contribute to the cost of the services being provided. Clients contribute a weekly program maintenance fee which is set at 17.5% of the maximum basic rate of pension. Any concerns about the fee can be discussed with the Coordinator/Case Manager and in accordance with our fee policy a reduction will be negotiated. No client will be declined service where there is genuine financial hardship.
