Centre Name and Location: Chantal (Dementia Day Centre), Waitara

 

Brief Description of Program/Centre:
Chantal (Dementia Day Centre) operates from Monday to Friday, 10:00 am to 15:00.

 

The centre provides opportunities of structured group activities designed to develop, maintain and support our client’s capacity for independent living and social interaction.  Chantal  has the capacity to care for up to 10 clients per day, with two to three dedicated, caring and qualified staff assisting at all times.  The service is suitable for people who would benefit from a smaller group setting. 

 

Chantal’s cottage atmosphere provides a relaxing yet stimulating environment.  The cottage provides secure areas both inside and outside, and has been designed in accordance with The University of Sydney Dementia Design Guidelines.

 

Clients are able to access a range of diversionary activities, such as music, art, craft, gardening and indoor and outdoor games.  Outings and day trips which are planned and appropriately risk assessed are also provided.  Our Younger Onset Dementia group meets every Wednesday. This active group heads out to various activities within the community, as well as the provision of centre-based programs .A group favourite in summer is participation in the Sailability program.

 

Transport to and from the centre can be provided by a group bus.  Clients arrive to a welcoming cup of tea followed by group exercises and a variety of group and person-centred activities. . Nutritious and varied home made meals are planned in advance to ensure our clients are well nourished. Experienced and caring staff ensures the personal needs of our clients are attended to at all times.

 

The centre provides much needed respite to carers from their caring role with the knowledge that the care recipients attending the centre needs are being met. Experienced staff are also available to assist clients and carers with direct support and information enabling access to other services provided by Catholic Community Services whilst at the centre.

Eligibility Criteria:  People in the community with diagnosis of Dementia. 

 

Cost to Clients: $15.00 per day = $5.00 per day (transport)
