Brief Description of Program:
A ComPacks a short term non clinical case managed package of community care available for people being transferred home from participating NSW Public Hospitals. ComPacks offers a safe, supported, transfer home from hospital and implemented sustainable support to prevent avoidable readmissions.

Funded By: NSW Department of Health 

Eligibility Criteria: Client needs to be discharging from a participating NSW public Hospital and requiring case management and two support services.  

Cost to Clients: $84, however ComPacks not declined based on inability to pay.
